Acknowledgement of Company Policies

I understand that I am not an employee of Loving Home Care until I have met with a family contracted with the DDD for services and both the family and myself have given written notification of intent to provide services.

I understand I will not be paid for the initial meetings with families interested in receiving care. I understand this is part of the interview process of the Job. I also understand that I am not to start working directly after and on the same day as the meeting. Some families will ask to stay and work right away; however, it is not permitted.

I understand I will not begin working with a family until I have received a “Good To Go” status from Loving Home Care. A Good to Go Status will be given when all parties agree to services and all the necessary paperwork has been completed. If I begin work before the "Good to Go" I may not be paid for the services.

I understand that the State of Arizona is switching to an Electronic Visit Verification system. If I opt out of the electronic system, I will need to print and mail my timecards two times per month at my own mailing expense in order to receive payment for services.

I understand that when providing Habilitation services, I can only bill up to 3 hours at a time separated by a break or at least one hour of Respite services.

I understand that I need to enter my hours regularly, and if I wait until the last minute to log hours, I may not get paid on the scheduled pay date.

I understand I may be asked to drive; however, I am not required to drive unless there is an emergency.

I understand that if I choose to drive my client, there are NO reimbursements from the DDD or Stepping Stones Care LLC.

I understand that in order to be a driver, I must keep current insurance and registration on the vehicle and use a checklist provided by Stepping Stones Care to conduct a basic safety inspection of the car annually.

I understand that sick time must be used for a qualified reason, such as sickness, a doctor's appointment, or a sick child or relative.

I understand that the use of sick time must be announced at the time of and on the same day of the shift in which the sick time is replacing.

I understand that I can only use the amount of sick time that I would normally use for that day. For example, if it is a 4-hour shift, then I can only take 4 hours of sick time for that regularly scheduled day.

I understand that I cannot use more hours than normally scheduled to work for that day as well as week. Example: If you are authorized for 20 hours per week of service you can only be paid for the hours actually worked plus sick time not exceeding a total of 20 hours combined.

I understand that sick time is not paid out as someone decides to exit the company. It is not considered vacation pay or paid leave.

I understand that the member’s guardian will need to state in writing the shift that was missed to be reimbursed by using the sick time hours.

I understand that parents or guardians will be asked to sign off on the Sick Time hours and verify they fit the guidelines above.

I understand that Loving Home Care sick time policies are in conjunction with the sick time laws of the State of Arizona.

Loving Home Care may choose to offer overtime to qualified providers only based on services they provide and longevity with the company. This will be determined case by case.

I will not go over 40 hours in one week unless pre-approved by Loving Home Care management. The work week shall begin on Monday and end on Sunday.

I understand I may be providing services in homes that use cameras or operate vehicles with surveillance systems. This is for everyone's safety, including my own.

I understand that in order to transport my client, I must first obtain clear, detailed, and specific instructions from the guardian pertaining to when, where, and how the member is to be taken anywhere, and what additional precautions may be necessary for the protection of each client's health and safety.

I understand that proper restraints must always be used when transporting, including seat belts and age-appropriate child car seats.

I understand that good hygiene practices - including washing hands frequently with soap and water for at least 20 seconds, wiping down frequently-touched surfaces, wearing a properly-fitted face mask, social distancing, and avoiding crowds - can all contribute to preserving the health and well-being of everyone, but are especially critical when working with immunocompromised clients.

I understand that any serious incidents involving a client (those that involve risk of immediate life-threatening danger or substantial damage to property) must be reported to Loving Home Care staff within 24 hours.

I understand that all other incidents which have the potential to affect the health or well-being of the member or pose a risk to the community must be reported to Loving Home Care staff by the close of the next business day.

I have read the DDD's Introduction to Incidents and Incident Reporting expectations at the link below and can identify categories and examples of incidents as well as the proper time frames for reporting.

I understand that sick time of 3 consecutive days requires a note from a doctor.

I understand that I am not allowed to work with non-DDD members at the same time as my client. This includes my children as well as non-DDD children and siblings of the DDD member receiving care.

I understand that client information such as name, location, contact information, and health information is protected by law and should not be revealed to unauthorized recipients per the HIPAA (Health Insurance Portability and Accountability Act of 1996) https://www.hhs.gov/hipaa/for- professionals/privacy/index.html. Disclosing such information is punishable by possible dismissal and prosecution.

I understand that clients introduced to me by Loving Home Care are proprietary to Loving Home Care. Your client CANNOT be moved to another agency without the advanced consent of Loving Home Care management, and any attempt to do so will be met with refusal to release the client, service hours, and immediate dismissal and prosecution. Client information is protected by law and service hours must be released before a client can be picked up by another agency.
