Have a clean motor vehicle driving record (reviewed semi-annually). Provide proof of insurance, valid vehicle registration, and valid driver’s license. Obey all traffic laws including but not limited to speed limit, complete stop, and following all road laws and advisory signs. Help the passenger in and out of the vehicle and always secure the passenger with a safety belt. Refrain from using mobile phone device while operating vehicle. Use designated pickup and drop off locations.
It is against Loving Home Care’s policy to use unsafe employee vehicles to transport members. It is the responsibility of the employee to keep the vehicle in proper and safe condition. The general checklist will be used to ensure the vehicle standards have been met and employee vehicles are good to go including a check of the climate control system. The checklist will be used upon hire and updated semi-annually. If at any time should an employee find any of the items listed on the vehicle checklist to be subpar, he/she shall not use the vehicle to transport members until the vehicle is repaired.
I have read and understand Loving Home Care Policy regarding vehicle transportation. I understand that it is my responsibility to only provide transportation in safe vehicles that meet the requirements outlined on the Vehicle Checklist. I understand that I cannot provide services if my vehicle is not insured, the vehicle registration is expired, or my driver’s license expires/ is suspended/ or revoked. Loving Home Care employees may use personal vehicles to transport members as long as the vehicle meets safety requirements and the driver meets standard requirements.
